To Get Started

To get started, call our Administrative office at 224-329-0090 or complete the “Request Information or Appointment” Form below.

We’ll ask you for basic information: your goals, availability and payment information. This will help us match you to the staff member who can best support your needs. This call typically takes about 10 minutes.

You can also call or email any or our therapists or consultants directly for more information or to make an appointment with them directly. You will find their phone and email info in their Bio on our Team page.

    Please keep in mind that communications via email over the internet are not secure. Although it is unlikely, there is a possibility that information you include in an email can be intercepted and read by other parties besides the person to whom it is addressed. Please do not include personal identifying information such as your birth date, or personal medical information in any emails you send to us.

    What to Expect

    Our office staff will reply to your call or email to answer any questions you may have about scheduling, payment, insurance, or Telehealth details.

    Once you make an appointment we will email you an invite to our secure and confidential client portal so you can complete all of the new client paperwork online prior to your first appointment. If you prefer to not use our secure client portal, paper-based forms can be completed at the beginning of your first appointment.

    The first time you meet, your therapist or consultant will partner with you to determine your customized goals. In future sessions, you’ll periodically review those goals together.

    Fees, co-payments, and payments toward deductibles are due at the time of service.

    Counseling Clients: If you plan to use your health insurance benefits, we encourage you to check with your insurance provider about your coverage for behavioral health services. Understanding your insurance benefits is your responsibility. Your member services phone number is usually found on the back of your insurance card. Please bring your insurance card and ID to the first appointment if you do not upload them in the portal.

    Upon completion of your new client documentation, our staff may contact your insurance company to verify your benefits. Once verified, Mindset will submit in-network insurance claims on your behalf.
